 - New k8s_auth_mount attribute (default 'kubernetes') lets login_k8s reach
   a Kubernetes auth method mounted at a non-default path
 - New soft_delete_secret (level 1, reversible), undelete_secret and
   destroy_secret (level 2) cover the rest of the KV v2 delete ladder;
   delete_secret stays the irreversible level 3 and its POD now says so
 - health returns the health hashref for sealed, standby and uninitialised
   servers too; undef means only that the server gave no usable answer
 - read_secret and read_secret_metadata take an optional version => $n to
   read a specific KV v2 version (GET data/...?version=N)
 - POD and README describe the 404 soft-miss contract for every method

 - secret_exists now propagates a 403 (permission denied) instead of reporting
   the path as absent; only a real 404 is treated as "does not exist"
 - add read_secret_metadata to expose the data.metadata block (version,
   created_time, destroyed) from a KV v2 read; read_secret's return shape is
   unchanged
