Add SelfServiceCustomFieldGroupings configuration variable to allow cfs groupings in SelfService

To install RT::Extension::ConditionalCustomFields, copy and paste the appropriate command in to your terminal.

cpanm RT::Extension::ConditionalCustomFields

perl -MCPAN -e shell
install RT::Extension::ConditionalCustomFields
