Changes for version 0.99 - 2006-10-04

Fix for Apache children always starting at index 0 and overwriting shared file if under app failover.
Simplified logic of _readsharedfile().
Changed and simplified execute(): now it always both reconnects and reexecutes a statement in case of hard failure (it used to only reconnect).
