Changes for version 2.030506

For Pari.xs, redefine away strtoi to avoid a conflicting declaration on NetBSD included via perl.h. Define PTRV (if needed) and include casts to PTRV when using INT2PTR (otherwise may fail on NetBSD). How comes that obsolete versions of INT2PTR survived there???
