Add subcommands: cancel-orders, cancel-all-orders.
open-orders: By default now show open orders from all markets, unless filtered by --pair. Add filter --type.
cancel-order: Add dry-run mode.
[Bugfix] Reenable xlm_idr, VIP reports some markets as invalid in get_open_orders() but they exist nevertheless.
