Changes for version 4

Minor POD fix.
Fix bug where '/' and '/index' were being treated differently.
Under some circumstances, $c->req->action wasn't containing the full path information we required.

To install Catalyst::Plugin::Breadcrumbs, copy and paste the appropriate command in to your terminal.

cpanm Catalyst::Plugin::Breadcrumbs

perl -MCPAN -e shell
install Catalyst::Plugin::Breadcrumbs
