Fixup builds for 5.28.x where there is already a stadtx_hash.h included in the build process via perl headers. Fixup documentation of fetch algorithm.

cpanm Algorithm::MinPerfHashTwoLevel

perl -MCPAN -e shell
install Algorithm::MinPerfHashTwoLevel
